The Registered Nurse Care Coordinator is considered an expert clinician and case manager whose nursing practice is characterized by being resourceful, flexible, innovative and self-directed. His/her approach to patient and family care/caregiver interactions mirror an inclusive practice of health and well-being. The Registered Nurse Complex Care Coordinator operates from an in-depth understanding of the importance of total health management and supports the patient and family/caregiver choices. The Registered Nurse Care Coordinator collaborates with physicians, advanced practice providers, and ancillary health care team members to coordinate system and community resources to maximize advocacy for patient and family care in achieving the most effective outcomes. The Registered Nurse Care Coordinator Program has grown into a very successful program within Waukesha County.
This RN Care Coordinator role specifically helps manage the Next Generation ACO (Medicare fee for service) population in the post-acute setting and supports the CMS Skilled Nursing Facility waiver process.
- Wisconsin Registered Nurse License required.
- BLS certification required.
- Bachelors degree in Nursing required or completed within 1 year of being in the role.
- Masters degree in Nursing preferred.
- 5 years of hospital-based nursing experience, adult primary care clinic or home care experience required.
- Case management, team lead or other nursing leadership experience preferred.
- Experience / knowledge of orthopedic patient population valued.
