Fully panelled nurse practitioner rendering services to all ages under a collaborative agreement with a designated PHMG physician. Responsibilities include: assessment, diagnoses, treatment and when necessary referral to primary care or specialty. Primary focus includes assessment and treatment of the ill or injured worker, meeting the needs of local employers, providing care for their workforce. Included in total worker health are onboarding physical exams and screenings including Department of Transportation examinations, episodic or urgent care visits and wellness consultations, education and Annual Wellness exams. Ensures systematic, ongoing data collection with relevant documentation, in accordance with the PHMG standards, the State of Wisconsin, PHMG Scope of Practice and Collaborative Agreement with the designated physician.
Preferred experience: Occupational Health, Employee Health, Family Practice, Urgent Care, Retail Health, 2-5 years experience however will entertain other candidates
Must complete DOT certification within 6 months of hire
Cerification in Occupational Health within 3-5 years
